In consideration of being allowed to participate in any way in G4 Archery events and activities, I acknowledge,
appreciate, and agree that:
1) The risk of injury from archery and other known and unknown events and activities and/or the use of the related
buildings, structures, equipment, automobiles, firearms, weapons, ATV’s, boats, tree-stands, roads, bodies of water, land and all other real and personal property whether owned by G4 Archery or others is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the
risk of serious injury does exist; and,
2) I acknowledge and agree that the use of archery equipment, firearms and other weapons by myself or others on premises or otherwise are inherently dangerous and high-risk activities whether such archery equipment, firearms or weapons are discharged by myself or others; and,
3) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
4) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
5) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS G4 Archery LLC., Tyne Industries LLC., Precision West Exteriors LLC., JH2 Construction LLC., its officers, directors, officials, agents, employees, volunteers, members, guests, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of real property and personal property used to conduct the events and activities (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or
damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, TO THE FULLEST EXTENT PERMITTED BY LAW. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS
BY AGREEING, AND AGREE FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
I grant permission to G4 Archery, LLC. hereinafter known as the “Media” to use my image in photographs and videos
for use in Media publications.
I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
I am the parent or legal guardian of the below named child. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to
do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities recommend practicing social distancing.
I further acknowledge that although G4 Archery officials have put preventative measures in place, they are unable to
guarantee that my child(ren) will not become infected with the Coronavirus/COVID-19.
I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the
actions, omissions, or negligence of myself, my child(ren) and others, including, but not limited to, staff, and other members of the G4 Archery community and/or their families.
I am voluntarily seeking attendance for myself at G4 Archery and acknowledge that in doing so, I am increasing my risk of exposure to the Coronavirus/COVID-19.
I acknowledge that I must comply with all set procedures to reduce the spread while attending events at G4 Archery. I commit to the following expectations of G4 Archery:
* If I begin showing symptoms including, but not limited to, cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell, I
will notify G4 Archery at 503-615-8008.
* If I, or anyone I have been in direct contact with receive a positive test for Coronavirus/COVID-19, I will notify
G4 Archery at 503-615-8008.
* I will follow all CDC, OHA, Washington County and G4 Archery recommended guidelines as much as possible to limit my exposure to the Coronavirus/COVID-19.
* I understand that there may be people wearing masks or not depending on medical conditions or health concerns. I hereby release and agree to hold G4 Archery harmless from, and waive any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself, children,and/or property that may be caused by any act, or failure to act of G4 Archery, or that may otherwise arise in any way in connection with any services received from G4 Archery. I understand that this release discharges G4 Archery from any liability or claim that I may have against them with respect to any bodily injury, illness, death, medical treatment of myself, children, family, or property damage that may arise from, or in connection to, any services received from G4 Archery. This liability waiver and release extends to the company together with all board members, partners, and employees.
